Overview

Acute Disseminated Encephalomyelitis (ADEM) is a rare, inflammatory condition that affects the central nervous system (CNS), particularly the brain and spinal cord of patients with an infection after they are administered the vaccines. Symptoms Causes

Symptoms:-

  • Unconditional fever, headache, and nausea.

  • Seizures.

Causes:-

The exact reason for ADEM, though, is unexplored, the outcome of an autoimmune effect is believed to be the reason which, in multiple cases, mistakenly attacks the myelin in the CNS due to viral or bacterial infections such as measles, mumps, rubella, or even influenza as the immune system of the patient, while fighting off the infection, gets weak. Diagnosis Tests

Diagnosing ADEM can be challenging due to its resemblance to other demyelinating conditions, such as multiple sclerosis (MS). Key diagnostic steps include:-

  • Neurological Exam - A physical and neurological exam can help identify the affected parts of the CNS based on observed symptoms.

Management & Treatment

Treatment:-

  • High-dose Corticosteroids - The mainstay treatment for ADEM is intravenous corticosteroids, such as Methylprednisolone, which help reduce inflammation in the CNS.
  • Intravenous Immunoglobulin (IVIG) or Plasma Exchange - These treatments may be used if a patient does not respond to corticosteroids. Both therapies aim to modulate the immune system and reduce the attack on myelin.
  • Rehabilitation - Physical, occupational, and speech therapy may be necessary for patients who experience significant neurological impairments due to ADEM.
  • Monitoring - Follow-up MRIs are often required to monitor the progression of the disease and ensure that inflammation is resolving.

Prevention

Since ADEM is usually triggered by infections, practicing good hygiene and ensuring up-to-date vaccinations can help reduce the risk of infections that might lead to ADEM. However, ADEM itself cannot be entirely prevented, as it arises from unpredictable immune responses.

Outlook/Prognosis

The prognosis for ADEM is generally positive, especially when treated promptly. Most patients recover fully or nearly fully within a few weeks to months. Some may experience lingering symptoms, such as minor weakness or cognitive changes. Rarely, ADEM can lead to permanent neurological damage or death if severe inflammation is not controlled.
