Overview

Arterial dissection, specifically carotid and vertebral artery dissection, occurs due to an incision caused in the wall of either the carotid or vertebral arteries, which supply blood to the brain, and develops a false lumen where blood usually pools and restricts blood flow, forming a blood clot. Such conditions contribute to the emergence of different complications such as stroke in the people, especially in younger adults. As far as Carotid Artery dissection is concerned it affects the arteries in the neck, while vertebral artery dissection impacts the arteries in the spine of patients.

Symptoms & Causes

Symptoms:-

The symptoms of arterial dissection can vary but often include:-

  • Sudden, severe headache (especially at the back of the head or neck).
  • Neck pain or stiffness.
  • Transient vision loss or double vision.
  • Difficulty speaking.
  • Weakness or numbness on one side of the body.
  • Tinnitus (ringing in the ears).
  • Dizziness or balance issues.

Causes:-

Causes of arterial dissection can be…

  • Trauma or injury to the neck (e.g., whiplash, sports injuries).
  • Genetic conditions (such as Ehlers-Danlos syndrome).
  • Hypertension (high blood pressure).
  • Spontaneous causes without obvious triggers.
  • Certain medical conditions that affect blood vessels, such as Fibro-muscular dysplasia.

Diagnosis & Tests

Diagnosing arterial dissection involves a combination of medical history evaluation, clinical symptoms, and imaging tests. Key diagnostic methods include:-

  • Magnetic Resonance Imaging (MRI) and Magnetic Resonance Angiography (MRA) - These imaging techniques provide detailed images of blood vessels to identify the dissection.
  • Computed Tomography Angiography (CTA) - CTA can quickly identify any tears in the arteries or blood clots that may have formed.
  • Ultrasound - In cases of carotid artery dissection, an ultrasound can detect abnormal blood flow.
  • Digital Subtraction Angiography (DSA) - A more invasive test used in some cases to get highly detailed views of the arteries.

Management & Treatment

The primary goal in managing arterial dissection is to restore normal blood flow and prevent strokes. Treatment depends on the severity of the dissection and may include:-

  • Antiplatelet or anticoagulant medications (like aspirin or heparin) - These help prevent blood clots from forming.
  • Thrombolysis - In cases where a stroke occurs, clot-busting drugs may be used to dissolve the clot.
  • Endovascular procedures - In severe cases, stenting or surgery may be required to repair the artery.
  • Surgery - Rarely, surgery may be needed if the dissection is causing severe complications or if less invasive treatments are ineffective.

Prevention

While not all cases of arterial dissection can be prevented, certain steps can reduce the risk:-

  • Maintain healthy blood pressure levels through lifestyle changes and medication.
  • Avoid trauma to the neck through proper protective gear during physical activities or contact sports.
  • Manage vascular health by quitting smoking and controlling other risk factors like cholesterol levels.
  • Regular check-ups if you have a genetic predisposition to blood vessel issues.

Outlook/Prognosis

The prognosis for individuals with arterial dissection varies depending on the severity of the condition and how quickly treatment is initiated. Many patients recover fully with appropriate treatment, although some may experience long-term effects, such as recurrent headaches or neurological symptoms. Early detection and prompt treatment significantly improve outcomes.

Living with Arterial Dissection

Living with an arterial dissection requires ongoing medical follow-up to monitor the condition and prevent further complications. Patients may need to make lifestyle adjustments, such as:-

  • Regular physical activity with precautions to avoid strain on the neck.
  • Medication adherence to prevent blood clots or control blood pressure.
  • Routine neurological check-ups to assess any long-term effects or risks of recurrence. For most people, a full recovery is possible, although some may need rehabilitation therapy if a stroke occurred.
