What is Carpal Tunnel Release?

Carpal tunnel release is a surgery that addresses and possibly cures carpal tunnel syndrome, a painful condition. Healthcare providers once believed that carpal tunnel syndrome was caused only by an overuse injury or a repetitive motion performed by the wrist or hand, typically while working.

Where is Carpal Tunnel Release Surgery Performed?

The carpal tunnel houses the median nerve, the cable that crosses the wrist and allows the fingers to flex. This median nerve passes from within the muscles of the forearm through a small, rigid channel formed by muscles and bones lying alongside the very bottom of the hand.

Why might I need carpal tunnel release surgery?

The only indication for carpal tunnel surgery is a diagnosis of carpal tunnel syndrome. However, your healthcare provider will want you to undergo some nonsurgical treatments first. These may be in the following form:

  • Over-the-counter pain medicines
  • Physical therapy
  • Changes to the equipment used at work
  • Wrist splints
  • Injections of steroids into the wrist to enable the reduction of swelling and pain in the area

The reasons that a health care provider may advise a person to undergo carpal tunnel release surgery may be as follows:

  • The nonsurgical treatments for carpal tunnel syndrome do not help alleviate the pain.
  • The health care provider administers an electromyography and conduction test of the median nerve. They then confirm that you are suffering from carpal tunnel syndrome.
  • The muscles of the hands or wrists are feeble and shrinking. This is due to severe pinching of the median nerve.
  • This has been for a long time with no relief concerning carpal tunnel syndrome symptoms.

How to Prepare for the Carpal Tunnel Release Surgery?

Preparation for carpal tunnel surgery is the same as that for any other surgery. Therefore, several tests are taken before the treatment. These tests usually happen 3 days before the procedure. Most probably, you will need the following:

  • EKG
  • Chest and arm X-rays
  • Blood and urine tests
  • The surgeon may require additional tests based on your condition. The surgeon would also guide you on common medications to stop the pain before the procedure.
  • Know whether you can take your standard drugs the night of the surgery or the morning after. You need to abstain from eating for six hours before surgery.

How is Carpal Tunnel Release Surgery Done?

To make you feel more comfortable during the procedure, you are given a local anesthetic medication. Although you will be fully awake, you will also need drugs that will make you relax. A small surgical incision is made in the front of your hand just above the wrist.

Next, the ligament covering the carpal tunnel is cut; now, the nerves are under less tension. Sometimes, tissue covering a nerve must also be released. Sutures will then stitch the skin or tissue beneath your skin.

The doctor will make a small incision close to your wrist, nearer the bottom of your palm. He will then insert the endoscope through your wrist. An endoscope is a long tube with a light and camera.

This procedure takes 15 to 30 minutes. You may experience some pain or unease as the anaesthetic starts to wear off. However, the doctor can prescribe some medicines to help.

What is the risk of carpal tunnel release?

Risks associated with this surgery are:

  • Medicine allergies
  • Bleeding
  • Infection
  • Median nerve injury or any of the nerves that the median nerve gives off.
  • Ache and numbness around the hand
  • Injury to another nerve or blood vessel (artery or vein) in a few cases
  • Tenderness around the scar
  • Worsening pain afterward

What should happen before a carpal tunnel procedure?

You should do the following before the surgery:

  • Tell your surgeon about any medicines you are taking. This includes medicines, vitamins, supplements, and herbal remedies you obtained over the counter.
  • You may be instructed to stop taking certain medicines that temporarily thin your blood. Examples include aspirin, ibuprofen (Advil, Motrin), naproxen (Naprosyn, Aleve), and others.
  • Tell your surgeon which medicines you will continue to take on the day of surgery.
  • If you smoke, quit. Smokers may meet with a smoking cessation counselor before surgery. Smoking delays healing.
  • Inform your surgeon about any cold, flu, fever, herpes outbreak, or other illness. If you become sick, you may have surgery postponed.

On the day of surgery:

  • Follow any instructions as to whether you are to stop eating or drinking before surgery.
  • Take any medicines you are asked to take with a small sip of water.
  • Follow instructions as to what time to arrive at the hospital. Make sure you get there on time.

What happens after a carpal tunnel procedure?

This surgery is performed on an outpatient basis. You will not need to spend time in the hospital.

You will probably have your wrist in a splint or heavy bandage for up to a week after surgery. It is best to keep it on until you visit your first surgeon following surgery and keep the area clean and dry. Once you've removed the splint or bandage, you will likely start motion exercises or participate in a hand therapy program.

How long does carpal tunnel surgery take to recover from?

Most patients can regain their daily activities in just a few days after surgery. Sporty or gym-related activities may take about four to six weeks to regain.

How long is carpal tunnel surgery?

Generally, carpal tunnel surgery takes ten to 20 minutes.

How long should I wait to drive after carpal tunnel surgery?

The return to driving after carpal tunnel surgery depends on several factors, including the type of surgery the patient underwent, whether it was an open or endoscopic carpal tunnel release, whether it was unilateral or bilateral, and the level of post-operative pain. It can take as little as a week to about two weeks.
