What is Ossiculoplasty?

Ossiculoplasty is an advanced surgical process to rebuild or mend the fractured middle ear ossicles (small ear bones) to regain hearing loss due to disruption of the ossicular chain. The three ossicles in the middle ear — hammer (malleus), anvil (incus), and stirrup (stapes) — pass the sound in the form of vibrations from the eardrum to the inner ear. Fixation, discontinuity, or destruction of these bones results in conductive hearing loss.

The main objective of Ossiculoplasty is to:-

  • Restore the conduction pathway of sound in the middle ear.
  • Repair or rebuild the ossicular chain with the patient's bone (autograft) or a prosthetic device (allograft).
  • Enhance hearing ability in patients suffering from middle ear damage.

Ossiculoplasty is usually performed under the Otology department of ENT Surgical Treatment Services and is highly effective in improving hearing in patients who have developed hearing loss due to damage to the ossicles.

Why is Ossiculoplasty needed?

Ossiculoplasty Surgery is advised by doctors when the ossicles (middle ear bones) are broken, eroded, or injured due to middle ear conditions. It can be understood by going through the primary reasons mentioned here, focusing on why medical professionals suggest Ossiculoplasty:-

  • Chronic Otitis Media: Also known as chronic middle ear infection, Otitis media is the common cause of ossicular chain wear.
  • Middle Ear Development: Known as Cholesteroleatoma, middle Ear Development is random skin formed by eardrum perforation or a long-standing infection that damages the bones of the middle ear.
  • Ossicular Chain Disorder: In other instances, trauma to the ear, head injury, or surgical complications may cause a break or discontinuity in the ossicular chain so that the sound is not transmitted to the inner ear. Ossiculoplasty is done to reestablish the connection between the ossicles through natural bones (ossicle graft) or prosthetic implants.
  • Congenital Ossicular Abnormality: Some people born with deformed or abnormal ossicles have hearing loss at birth, for which ossiculoplasty or surgery is the only option to restore their ossicular chain and enhance hearing capacity.
  • Stapes Fixation or Ankylosis: If the stapes bone (middle ear bone) becomes fixed or does not move because of otosclerosis or trauma, it may cause hearing loss.

Types of Ossiculoplasty Operation

The following are the ways adopted by doctors for operations:-

Partial Ossicular Replacement Prosthesis (PORP)

  • It opts if the malleus and stapes bones remain intact while the incus bone (anvil) is missing or impaired.
  • A Partial Ossicular Replacement Prosthesis (PORP) is employed to fill in between the malleus and the stapes so that sound conduction is possible.

Total Ossicular Replacement Prosthesis (TORP)

In three ossicles (malleus, incus, and stapes) are damaged a Total Ossicular Replacement Prosthesis (TORP) remains the option under which a prosthetic apparatus is used to send sound from the eardrum to the internal ear.

Autograft or Homograft Ossiculoplasty

If partially damaged ossicles are present in the patient, the surgeon might reconstruct them using the patient's natural ossicle bone (autograft) or donor bone (homograft).

Process of Surgery

  • Before ossiculoplasty, the patient is put on anaesthesia.
  • The persisting problem is Analyzed.
  • A thorough audiometry (hearing test) is done through an MRI /CT scan.

Post-Surgery Actions

  • After surgery, patients are discharged within 24-48 hours, and they recover faster and perform routine activities of life efficiently. However, may feel slight dizziness.
  • You must not travel, swim, or lift a heavy object for a month. Besides, you must protect your ear from water, dust, or noise and avoid heavy exertion or physical exercise to safeguard your head from impact.
