Hirschsprung disease is a congenital condition (present at birth) that affects a child’s large intestine (colon). In this condition, certain nerve cells that control bowel movement are missing in a part of the intestine. Because of this, the affected segment cannot relax and push stool forward properly, leading to a blockage of stool in the bowel.
Hirschsprung disease most commonly affects newborns and infants, but in mild cases, it may be diagnosed later in childhood. Without treatment, it can cause severe constipation, bowel swelling, and serious infections.
In a healthy intestine, nerve cells (called ganglion cells) help the bowel muscles contract and relax in a coordinated way to move stool forward. In Hirschsprung disease, these nerve cells are absent in the affected part of the colon. As a result, stool gets stuck behind this segment, causing the normal part of the bowel to stretch and swell. Over time, this can lead to chronic constipation, abdominal distension, and bowel obstruction.
The symptoms often begin soon after birth. One of the earliest and most important signs is
Failure to pass meconium (the first stool) within the first 24 to 48 hours of life.
Newborns may develop a swollen abdomen, vomiting (sometimes green or yellow), poor feeding, and irritability.
In babies and older children, symptoms can include ongoing constipation, a bloated belly, trouble gaining weight, slow growth, and times of diarrhea with bad-smelling stools.
Some children may get repeated gut infections or fever, which can be signs of a serious problem called enterocolitis.
Hirschsprung disease occurs when nerve cells in the intestine do not develop as they should during pregnancy. The exact cause is not always clear, but genes play a role. It is more common in boys than girls and sometimes happens along with other conditions, such as Down syndrome. It is not caused by anything the mother eats or by anything parents do after birth.
If Hirschsprung disease is not treated, it can cause a serious blockage in the bowel, ongoing constipation, and poor growth. One of the most serious problems is Hirschsprung-associated enterocolitis. This can cause fever, severe diarrhea, a swollen belly, and can be life-threatening if not treated quickly.
Doctors may suspect Hirschsprung disease based on a child’s symptoms and a physical exam. An X-ray of the belly can show swollen areas of the bowel. A special enema test can help find the narrow part of the colon. The most reliable test is a rectal biopsy, which checks if nerve cells are missing in the bowel wall. Finding the disease early helps prevent problems.
The main treatment for Hirschsprung disease is surgery. The goal is to take out the part of the intestine that does not have nerve cells and connect the healthy bowel to the anus so stool can pass normally. This is often done with a surgery called a pull-through surgery.
Before surgery, some children may need bowel washes and extra care to help reduce swelling and lower the risk of infection. After surgery, most children slowly start to have more normal bowel movements, but some may need time to get used to it.
After treatment, children need regular check-ups to monitor their bowel function, growth, and nutrition. Some may have short-term issues like loose stools, constipation, or trouble controlling their bowels, especially in the first few months after surgery. With good care and regular follow-up, these problems usually improve over time.
The long-term outlook for children with Hirschsprung disease is usually very good, especially if it is found and treated early. Most children grow and develop normally and have a good quality of life. Getting medical help early for symptoms like delayed stool passage, ongoing constipation, or a swollen belly is important for the best outcome.
