Known for high vascularity and several blood vessels, the Juvenile Nasopharyngeal Angiofibroma (JNA) is a rare, benign tumor primarily affecting adolescent males, generally between the ages of 10 and 25. It originates in the Nasopharynx, the hinder part of the nose, and exceeds the back of the throat. However, what makes it a problem of much concern is it being benign or non-cancerous, yet affecting the person as a tumor that grows aggressively, spreading to nearby structures, triggering problematic symptoms that consequently disrupt routine activities of the life of the affected person.

What are the symptoms of Juvenile Nasopharyngeal Angiofibroma

Following are the common symptoms of JNA that usually depend on the size and location of the tumor, which grows progressively:-

  • Nasal Obstruction - Difficulty breathing through the nose is often one of the first signs.
  • Frequent Nosebleeds - It is a a recurrent Epistaxis, or nosebleeds, considered a hallmark symptom due to the tumor vascularity
  • Facial Swelling is caused due to growth of tumor grows makes its swelling visible particularly around the cheeks/eyes
  • Hearing Loss or Ear Fullness - JNA can obstruct the Eustachian tube, leading to ear-related symptoms
  • Headaches and Vision Changes can pressure surrounding nerves and tissues, triggering headaches or eyesight impairments

Although the exact cause of JNA has yet not been ascertained, it is believed to be erupting with changes in hormones during puberty and affecting predominantly male adolescents but can't be linked congenital syndromes as there is no proof of it that substantiate the information.

How is Juvenile Nasopharyngeal Angiofibroma diagnosed

The following are the diagnostic options for JNA that usually begins with a physical examination, particularly of the nasal cavity, and if JNA is suspected, doctors may recommend imaging studies as well:-

  • CT or MRI Scans - These scans help to visualize the extent of the tumor and its impact on surrounding structures
  • Angiography - As JNA is highly vascular an angiogram, the imaging diagnostic option may be conducted to map the blood vessels supplying the tumor that suggests treatment option
  • Biopsy is avoided due to the high risk of bleeding

What are the treatment options for Juvenile Nasopharyngeal Angiofibroma?

Treatment typically requires surgery to remove the tumor but in condition of high blood flow, preoperative Embolization, a procedure to reduce blood supply to the tumor is conducted to minimize bleeding before surgery but in some cases, radiotherapy or endoscopic surgery may also be recommended, especially if the tumor is extensive or in hard-to-reach areas. Medications may also be recommended to control growth of tumor, though surgery remains the best option for JNA.

Can Juvenile Nasopharyngeal Angiofibroma be prevented?

Currently, there are no known preventive measures for JNA due to its unclear cause and rarity yet routine check-ups and monitoring symptoms like nasal obstruction or recurrent nosebleeds in high-risk age groups are believed to help in early detection and manage the condition.
