What is Dysmenorrhea?

Dysmenorrhea means painful periods, also called menstrual cramps. Besides pain in the lower belly, some women may feel sick to their stomach, tired, have a headache, feel dizzy, or have diarrhea.

Mild cramps during periods are common. But if the pain is so bad that it makes it hard to do normal things, you may need to see a doctor. Most cramps start just before or on the first day of your period and get better in 2 to 3 days.

What are the types of dysmenorrhea?

There are two types of dysmenorrhea:

Primary dysmenorrhea

  • Common type
  • Pain occurs without any underlying disease
  • Starts just before or at the beginning of periods
  • Pain is usually felt in the lower abdomen, back, or thighs
  • Improves within a few days

Secondary dysmenorrhea

  • Pain is caused by a health problem with the organs that help with having babies
  • Pain often begins before the period and lasts longer
  • Less common but usually more severe
  • Needs to be checked and treated by a doctor

What are the symptoms of dysmenorrhea?

Symptoms may include:

  • Cramping or throbbing pain in the lower abdomen
  • A feeling of pressure or heaviness in the lower belly
  • Pain in the lower back, hips, or thighs
  • Nausea or vomiting
  • Headache or dizziness
  • Fatigue

Pain usually begins 24–48 hours before periods and improves within 48 hours after bleeding starts.

What causes painful periods?

Menstrual cramps happen because of a chemical in the body called prostaglandin, which makes the uterus tighten. More prostaglandin means stronger tightening, which causes pain and cramps.

As prostaglandin levels go down after your period starts, the pain usually gets better.

Who is more likely to have dysmenorrhea?

You may be at higher risk if you:

  • Started periods before age 12
  • Are younger than 20 years
  • Have heavy or long menstrual periods
  • Smoke cigarettes
  • Have a family history of painful periods

Are painful periods dangerous?

Painful periods are not dangerous, but they can make life harder.

However, if cramps are caused by conditions like:

  • Endometriosis
  • Pelvic inflammatory disease (PID)
  • Fibroids

They may cause problems like trouble getting pregnant. This is why very bad or long-lasting pain should not be ignored.

How is dysmenorrhea diagnosed?

Your doctor will:

  • Ask about your symptoms and menstrual cycle
  • Do a check of your lower belly and reproductive organs
  • Take vaginal samples if needed

If secondary dysmenorrhea is suspected, additional tests may include:

  • Ultrasound – to examine uterus and ovaries
  • Hysteroscopy – to view the uterus
  • Laparoscopy – to check pelvic organs directly

How can menstrual cramps be relieved?

Medications

  • NSAIDs (ibuprofen, naproxen) reduce prostaglandins and pain
  • Acetaminophen if NSAIDs are not suitable
  • Hormonal birth control (pills, patch, ring) to reduce cramps

Home remedies

  • Heating pad on lower abdomen or back
  • Gentle massage
  • Regular exercise
  • Adequate sleep
  • Avoid caffeine, alcohol, and smoking

Can dysmenorrhea be prevented?

Dysmenorrhea cannot always be stopped, but healthy habits like regular exercise, eating well, and managing stress can help make it less severe.
