What is the Tetralogy of Fallot in children?

Tetralogy of Fallot is a heart disease in which babies are born with four abnormalities in the development of their hearts. This makes it hard for your baby's heart to deliver enough oxygen throughout the body. Usually, blood always follows a path through the heart with each heartbeat. The structure of the heart abnormality with the tetralogy of Fallot can lead to some oxygen-free blood moving into the aorta and out of the body rather than into the pulmonary artery to pick up oxygen. 

What are the four factors of Tetralogy of Fallot (TOF) in children?

Tetralogy of Fallot (TOF) is described by the following:

Ventricular septal defect (VSD): This is a hole between the heart's two lower chambers. That hole allows oxygen-poor blood to mix with oxygen-rich blood.  Pulmonary stenosis is the narrowing of the pulmonary valve and the arteries that lead to the lungs, making it difficult for blood to reach the lungs to pick up oxygen.

Right Ventricular Hypertrophy: The muscle of the right ventricle (lower right chamber) thickens because it's working harder to pump blood through the narrowed pulmonary valve. Overriding Aorta- The Aorta is the main artery leading out of the heart, is shifted and lies directly over the ventricular septal defect (VSD). This causes oxygen-poor blood to flow directly into the aorta rather than going to the lungs.

What are the symptoms of tetralogy of Fallot in children?

The severity of the tetralogy of Fallot symptoms depends on the degree to which blood flow is hindered from the heart to the lungs. Some of the signs could be:

  • Cyanosis: This is the most common symptom, appearing as a bluish shade on the skin, lips, and fingernails. It occurs because lower oxygen-rich blood is reaching these areas.
  • Tachypnea or rapid breathing: The body attempts to compensate for low oxygen conditions by increasing the breathing rate.
  • Tiredness and getting quickly exhausted during exertion: Children with TOF are likely to tire effortlessly during playtime or exercises due to their bodies not taking enough oxygen for exertion.
  • Clubbing: After some time, some children with TOF start developing clubbing, where the fingertips and toenails become widened and rounded. This is a result of long-term low-oxygen situations.
  • Heart murmur: A doctor can hear a whooshing sound through a stethoscope while listening to the heart, which reflects abnormal blood inflow.
  • Tet spells quenching events tend to prevail in children with TOF: During a tet spell, the child artificially squats down to ameliorate blood inflow to the lungs, which helps ease their breathing difficulties. These occurrences can be scary for parents, but knowing the signs can help you manage them. 

What causes Tetralogy of Fallot (TOF) in children?

Possible causes include:

A commodity in your child's DNA may have changed, but this change is doubtful to be caused by a parent. Parents who are born with heart defects do not pass this on to their children. However, if both parents have heart disease, the threat may be advanced.

  • A history of rubella, diabetes, alcohol use, and poor diet during pregnancy can heighten the risk of intrinsic heart disease in a fetus.
  • When pregnancy occurs over the age of 40, it may be considered a risk factor.
  • A very rare disorder known as phenylketonuria-accumulation of amino acids, can increase your child's risk of heart disease.

What are the diagnosis and tests for Tetralogy of Fallot (TOF) in children?

Doctors generally diagnose the tetralogy of Fallot during immaturity or early non-age through various tests, including:

  • An echocardiogram: It is an ultrasound of the heart to see its structure, function, and how well the heart is working.  Chest X-ray or CT- It provides a picture of the heart and lungs that shows the abnormal shape of a charge.
  • Pulse Oximetry: It measures the oxygen position in the blood. Before your baby can be discharged home, your doctor will attach detectors to your newborn's base or hands to measure their oxygen position. Electrocardiogram (ECG)- It is a test that records the electrical exertion of the heart, which at times might give some clues about abnormal heart measures associated with TOF.

What is the treatment for a child with Tetralogy of Fallot?

Surgery for tetralogy of Fallot returns inflow to normal. However, other problems can even arise, including dental care, which is very important because you may be at an advanced threat for heart infections (endocarditis). Antibiotics can help prevent infections during dental visits.  Babies with tetralogy of Fallot may develop and grow more slowly.

It could be necessary to seek physical, speech, and occupational therapy.  You may be at increased risk for arrhythmias. Your healthcare provider may need to assess your heart before you become pregnant, which places you at increased risk for delivery.  And as an adult, heart disease can also impair your ability to use specific methods of contraception.
