What is Peritoneum Cancer?

Peritoneal cancers are cancers that have developed on or moved to the lining of the abdominal cavity, which is called the peritoneum. These cancers may start as ovarian cancer, colorectal cancer, appendix cancer, mesothelioma, or a rare condition called pseudomyxoma peritonei.

Another medical name for these cancers is peritoneal carcinomatosis. This type of cancer can either begin directly in the peritoneum or spread there from other organs like the colon, appendix, or ovaries. When cancer is found in the peritoneum, it is considered an advanced stage (Stage IV).

Who is likely to get this cancer?

No one knows the exact cause of peritoneal cancer. It begins when cells in the lining of the abdomen change and begin to grow out of control. This can happen because of an existing cancer that has spread from another organ, or it can start as a specific type called primary peritoneal cancer.

What are the signs and symptoms?

In the early stages, people often do not notice any symptoms. When signs do appear, they are often vague and can be easily mistaken for other problems. Common symptoms include:

  • Pain in the abdomen.
  • Diarrhea or frequent constipation.
  • Feeling full very quickly after eating.
  • Swelling or bloating of the abdomen.
  • Unexpected weight loss or a lump in the belly.
  • Unusual vaginal bleeding.

As the disease progresses, fluid may build up in the abdomen (ascites). This can cause nausea, vomiting, and difficulty breathing. It is also very common to feel extremely tired.

In more severe cases, the cancer can cause blockages. A blockage in the bowel might require a stoma (a surgically created opening for waste to leave the body). A blockage in the urinary tract may require a stent or a nephrostomy tube to help drain urine from the kidneys to the outside of the body.

How is it diagnosed?

Doctors use several tests to find out if the cancer started in the peritoneum or spread from the liver, colon, stomach, or ovaries.

  1. Medical History and Physical Exam: Your doctor will talk to you about your symptoms and perform a physical exam. They may be able to feel fluid or small tumor nodules in your abdomen.
  2. Blood Tests: Doctors check for "tumor markers," which are substances in the blood that often increase when this type of cancer is present.
  3. Imaging: A CT scan or MRI of the abdomen and pelvis helps the doctor see the extent of the disease. A PET-CT scan may also be used to see if the cancer has spread to distant areas, such as the lungs.
  4. Laparoscopy: This is a minor surgery where a doctor makes small cuts to insert a camera and take small tissue samples (biopsies). This confirms the diagnosis and helps the doctor calculate the Peritoneal Carcinomatosis Index (PCI), a score that tells the doctor how far the cancer has spread and if surgery is an option.

How is it treated?

Treatment has changed significantly in recent years. Depending on the type of tumor, a combination of surgery and specialized chemotherapy is often used.

  • Cytoreductive Surgery and HIPEC: First, a surgeon performs "cytoreduction" to remove all visible tumors. This is followed by HIPEC (Hyperthermic Intraperitoneal Chemoperfusion). During HIPEC, a heated chemotherapy solution is circulated directly inside the abdomen. The heat helps the medicine kill any remaining microscopic cancer cells more effectively.
  • Systemic Treatment: You may also receive chemotherapy through an IV or in pill form before or after your surgery.
  • Other Therapies: For advanced cases, doctors may use Targeted Therapy (drugs that target specific cancer cells), Hormone Therapy, or Radiation Therapy to help shrink tumors or manage the disease.
