What is Ureteroscopy?

A ureteroscopy is a very specialised procedure that is minimally invasive in exploring and treating problems with the urinary tract, particularly the ureters, known as the tubes connecting the kidneys to the bladder. In most cases, ureteroscopy removes ureteral stones, diagnoses obstructions, and treats tumours. A skinny, flexible or rigid tube called a ureteroscope goes up through the urethra and bladder into the ureter, which the urologist uses to visualise treatment of the stented condition without incisions. It usually works as an outpatient, and post-operative recovery is faster than conventional open surgery.

When is Ureteroscopy Recommended?

Ureteroscopy is not a disease in itself but a treatment modality recommended for patients with symptoms showing urinary tract problems, such as:-

  • Severe or intermittent flank or abdominal pain
  • Blood in the urine (hematuria)
  • Urinary tract infections (recurring or chronic)
  • Difficulty or pain while urinating
  • Unexplained urinary obstruction

How Ureteroscopy Works

In general, ureteroscopy works under general anaesthesia or spinal anaesthesia. The procedure steps include:-

  • The medical experts insert a ureteroscope through the urethra and into the bladder through the ureter.
  • Visual inspection of the ureter and kidney collecting system.
  • If there is a stone:-
    • Extract small stones using a basket.
    • Laser lithotripsy (commonly with a Holmium laser) fragments larger rocks and then collects the disintegrated parts.
  • A biopsy may work if a tumour or stricture becomes visible.
  • A doctor may temporarily place a stent in the ureter for drainage and healing.

The overall procedure takes about 30 min to 90 min, according to the complexity.

  • Oral analgesics would well control pain.
  • No heavy lifting or vigorous exercise for 3-5 days.
  • Bring a follow-up appointment with the doctor for pathology review (if biopsies held) or stent removal within 1-2 weeks.

It's essential to immediately complete any antibiotic course and report any signs of fever, chills, or severe pain.

Does ureteroscopy hurt?

The ureteroscopy is done under local anaesthesia, so, at least during the procedure, there will not be any pain. After the procedure, there could be mild colicky pain.

How long should I stay in the hospital?

Most of the ureteroscopies are day surgery, so one has to go home on the same day.

What are the side effects of ureteroscopy?

The risks are infrequent but may include infection, bleeding, injury to the ureter, or discomfort from the stent.

Can ureteroscopy remove all kidney stones?

It is perfect for stones from the mid to the lower ureter or small renal calculi. PCNL or shock wave lithotripsy (SWL) may be necessary for larger or multiple stones.
